What you need for your entry form:

• Your contact details
• A hi-resolution image of your completed artwork
• Dimensions (cm) and weight (kg) of your artwork
• Brief Artist Statement about the artwork
• Artist CV (optional)

Please ensure to carefully read the 2025 Art Awards Terms and Conditions prior to completing the entry form. Entries that do not comply with these conditions will not be accepted.

Hard copy entry forms can be collected from the City of Belmont Civic Centre or provided upon request.
