From 5 December 2020 visitors to City of Belmont facilities will be required to record their details for WA Health Department COVID-19 contact tracing.
These facilities include but are not limited to Belmont Hub, Civic Centre, Operations Centre and community centres.
Attendance details can be registered by either scanning QR codes at the venue using the State Government’s SafeWA app or by providing your details to staff at the Civic Centre, Belmont Hub, or completing a hard copy form at the City’s community centres.
If COVID-19 is detected in the WA community, contact registers enable the WA Health Contact Team to quickly identify and assist anyone that may have been exposed to COVID-19.
